Thank you for attending a Child and Family Team (CFT) Meeting. We would like you to take a moment to complete this survey. It will take five to seven minutes to complete. Your answers will help improve the CFT Meetings.
The survey is optional. Except for your role in the CFT Meeting (child or youth, current caregiver, supporter, or placing agency worker), you may skip any survey question. We will not ask information that link you to your answers. No county staff, service providers, nor anyone on your team will access your individual answer. Responses will be analyzed by CDSS and shared as a whole with Counties, Tribes, and other partners as available to ensure responses are anonymous.
